We have made a number of forms available for immediate download from this site.

All the forms are in Adobe Acrobat PDF format. If you don't already have the free Adobe Reader, you can go here and download it, then return to this page to retrieve the form (s) you're interested in.

Out of School Hours Care	Registration form for applying for after hours care. There are two options available: You can download and print the PDF; Or you can download and print Word document; Both the print options require you to print out the document yourself, fill it in, sign it, and send it in to the school's mailing address. If you're not sure which document to download, choose the PDF, that will work with most modern computers (Macintosh, Windows, and Linux).	PDF

Here are some tips that should help make sure you get the form you want:

Normally, clicking on the form title or icon will simply display the form in your web browser if you're using a modern web browser, such as Internet Explorer 6 or later, or Mozilla Firefox 2 or later. If you wish to save the form on your own computer, right-click on the Acrobat image link and select "Save As..." in your browser's context menu. This will save the form on your own computer.

- If you're using an older, or text-only browser, you will be prompted to download the form without any other options.

- To save time (and your sanity!), save the downloaded form in your "My Documents" folder so you can find it again without having to search the whole computer!

- Don't worry about all the % (percent) signs in the names of the forms, that's just so that web browsers can handle the file names without breaking. When the file is on your computer, it will have the correct name, we promise!
